New Patient Registration

If you live within the catchment area for Castle Healthcare Practice and would like to register, please download the relevant forms below.

You need two forms for each person registering:

1. The adult form (patients aged 17 and over - click here) or the child form (patients aged 0-16 - click here)
2. The family doctor service registration form - click here

Please bring your NHS number, photo ID (Passport, Driving License, Visa) and proof of your address (Utility bill, bank statement etc) to reception.
 
Please note - If you are taking any regular medication you will need an appointment to speak a GP before a repeat prescription can be issued. 

New Patients - We are required to provide a named and accountable GP for all patients.
Yours is the Registered and Usual GP automatically allocated when registering at Castle Healthcare Practice. If you require more information please ask at reception.  This may not necessarily be the GP that you choose to see.

If you move outside of our Practice catchment area you will be sent a letter asking you to re-register at a Practice closer to your new home.

Changes to Personal Details

Please notify the receptionist of any change of address or telephone number. Please also let us have your mobile telephone number.  If you give consent for SMS messaging, we will contact you to send appointment reminders and other messages.

We do not accept permanent registration requests from patients who permanently reside outside of our catchment area.

Temporary Registration

You can register as a temporary patient if you're in the area for longer than 24 hours but less than 3 months.

If you'll be in the area for longer than 3 months, you'll have to re-register as a temporary patient or permanently register with the practice.

You can receive emergency treatment from a GP surgery for up to 14 days – for example, if you fall ill while on holiday. If your treatment will last longer than that, you'll have to register as a temporary or permanent patient.

To register as a temporary patient, download and complete the Temporary Resident Registration Form and bring a copy to reception.

NHS England Guidance for Temporary Residents

NHS England guidance for patients that are registered as a patient in England and staying away from their home address (within England):

• Routine appointment - Contact your own GP surgery and ask for a telephone consultation.
• If a face-to-face appointment is required, you will be directed to the nearest Urgent Care/Walk-In Centre. This applies to all patients, including students at home from university for holidays.
  • The nearest Urgent Care Centre is - Seaton House, London Road, Nottingham, NG2 4LA.

• If you require a repeat prescription you should contact your own GP surgery and ask for a prescription to be sent electronically to a pharmacy near you. You are able to ask a pharmacy for a 5 day emergency supply of medication, if required.

NHS App

The most efficient method of accessing online services is now via the free NHS App. Owned and run by the NHS, the NHS App is a simple and secure way to access a range of NHS services online, including ordering repeat prescriptions and accessing health information.

Online Service

Patients are also able to access online tools via the 'Online Services' page on our website. 

Online services do not replace traditional ways of contacting your GP over the phone or in person. They simply offer additional ways to interact with your GP, making it easier and more convenient.

For example, you can book and cancel appointments, order your repeat prescription, or look up your medications online.

In return, it's hoped the services will free up phone lines for people with no internet access, and enable GP practices to manage appointments and telephone calls more efficiently.

GP2GP

GP2GP allows patients' electronic health records to be transferred directly, securely, and quickly between their old and new practices, when they change GPs. This improves patient care by making full and detailed medical records available to practices, for a new patient's first and later consultations.
