New Patients

New Patient Registration

If you live within the catchment area for Castle Healthcare Practice and would like to register, please download the relevant forms below.

You need two forms for each person registering:

  1. The adult form (patients aged 16 and over – click here) or the child form (patients aged 0-15 – click here)
  2. The family doctor service registration form – click here

Please bring your NHS number and proof of your address (Utility bill, bank statement etc) to reception.
 
Please note – If you are taking any regular medication you may need an appointment to speak a GP before a repeat prescription can be issued. Please ensure you have a sufficient supply from your current Practice before registering.

New Patients – We are required to provide a named and accountable GP for all patients.
Yours is the Registered and Usual GP automatically allocated when registering at Castle Healthcare Practice. If you require more information please ask at reception.  This may not necessarily be the GP that you choose to see.

We welcome all patients living within our Practice area and operate a non-discrimination policy.  Click on the link below to see our Catchment Area.

If you move outside of our Practice catchment area you will be sent a letter asking you to re-register at a Practice closer to your new home.
